Associate degree or higher in Nursing
Valid Certified Cardiac Rehabilitation Professional (CCRP) certification
Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
Valid Driver’s License (DL)
All of the following valid certifications:
American Heart Association Basic Life Support (AHA BLS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Preferred
RN License Number (required for name clear)
Join Our Award Winning Client as a Start of Care (SOC) Nurse (RN) – Full-Time, Weekend Role. This position promotes coordinated and comprehensive care for patients through initial assessments, care planning, and direct nursing services. The ideal candidate brings strong clinical skills, attention to detail, and a collaborative spirit to support quality patient outcomes in a home health setting.
Complete start-of-care documentation for new admissions and post-hospitalizations
Assess patients for telemonitoring eligibility and integrate telehealth services into care plans
Collaborate with healthcare professionals to develop and implement individualized care plans
Monitor and report patient responses to treatments, medications, and any changes in condition
Educate and counsel patients and their families on care procedures and health status
Assign and guide Home Health Aides with detailed plans and instructions
Perform preventive and rehabilitative nursing procedures per agency policy
Maintain timely communication with supervisors and the home care team
Participate in staff meetings, audits, CEU training, and performance improvement initiatives
Adhere to state board CEU requirements and keep current on documentation standards
Ensure professionalism in all interactions with patients, families, and staff
Report daily to supervisor or scheduler and communicate any schedule changes
Flexibly respond to schedule changes and help meet staffing needs
Organize patient visits efficiently to minimize travel and meet weekly productivity goals
Expected to reach productivity equivalent of three (3) SOCs or ROCs per day, totaling six (6) points/day
Work holidays as needed
Ensure documentation is timely and accurate
Perform other duties as assigned by supervisor
Monday to Friday , 8:00 AM – 5:00 PM
Weekend focus
Regular travel required to meet business needs and patient locations
